Program for the dedication of a bronze sculpture of John L. "Jack" Swigert, presented at the United States Capitol, Washington, D.C., May 22, 1997, after Swigert's death. Ken Mattingly was the original command module pilot of the mission, but was replaced by John Swigert days before the launch due to exposure to the measles. Additionally signed by sculptor Mark Lundeen, Colorado politician Dennis Gallagher and NASA Flight Director Gene Kranz.
