Pennsylvania Medical Certificate for Samuel Moore of Cumberland County, New Jersey. Partly printed document filled-in in manuscript. With printed vignette of the Pennsylvania Hospital. With printed seal. Minor chipping to edges. Also signed by Philip Syng Physick and the managers of the hospital. Bold, dark signatures.
Rush, Benjamin (1745-1813), prominent early American physician; signer of The Declaration of Independence; published the First American chemistry and psychiatry texts.
Physick, Philip Syng (1768-1837), prominent early American physician and surgeon.
