Parker Surgeon in the Navy; counter-signed by Acting Secretary of the Navy, Daniel Ammen; embossed blue United States Navy Department seal at bottom, upper points creased. Line "of the grade of Lieutenant Commander in the Navy, to have relative rank as provided by law," added along right side in a secretarial hand. Creasing from original folds. 19 3/8 x 15 3/8 in. (492 x 390 mm).
